Please give your name, contact telephone number, 2024 USTA level. Players will be placed according to roster availability & past experience on league teams.
Captains will review this sheet before each upcoming season.

This form is to gather level of interest only. It is not a guarantee to play or be rostered, spaces will be limited on the team per the captain's discretion in making line-ups.
You must have a current USTA membership to participate in a USTA league and you will need to be rated appropriately. Players listed on a roster should not expect to play every week.

Please register interest no later than April 30th. League is slated to begin in June - August. Mondays 9AM: 2 lines start w/ 1 to follow.
